That depends on the quality, scope, and desired flexibility of the existing theme. We first check what's solid.

Standard app or custom feature?

A good standard app is often more economical. Custom development pays off for special logic, data ownership, or limits that keep getting in the way.

Headless Shopify too?

Yes, if content, frontend, and operations justify the additional architectural effort. Headless isn't an end in itself.
